TEAM MANAGER
(HOSPITAL DISCHARGE TEAM, SOUTH TYNESIDE)
£43,757 pa (A Market Force Supplement up to £47,578 may be considered for an exceptional candidate)

Serving a population of almost 150,000 in the heart of the North East, South Tyneside Council prides itself on providing excellent and efficient services to its residents.

Adult Social Care is changing in South Tyneside. We are a forward thinking Council that is developing innovative approaches to meet our challenges and are designing new ways of working that are person centred, including having different conversations that build on people’s strengths and assets.

With a low level of delayed transfers of care we are keen to lead the way in terms of innovation that will see us maintain this level of performance.

Providing both robust leadership and management of our Hospital Discharge Team within South Tyneside District Hospital, you will co-ordinate and affect timely discharges for patients with ongoing health and social care needs. You’ll have a successful background of multi-disciplinary and partnership working which will build on the excellent joint working relationships already established with health colleagues.

Candidates will need a deep understanding of excellent social work practice, and the rigour and management grip to work within the complexities of a multi-disciplinary hospital setting. You will be innovative and creative, open to ideas and committed to individual learning and development.

Our aim is to deliver practice to be proud of and lead social work change to ensure Adults within South Tyneside are supported to Live Better Lives. Therefore, if you are committed to adults having choice and control over their support, maximising opportunities for independence and well-being and wish to be part of an organisation that focuses on best practice and aspires to be the best, then this is the role for you.

For an informal discussion about this post please contact Sarah Parker, Service Manager, Adults and Integrated Care on (0191) 424 7475.

Interviews will be held on Friday 5 October 2018.

South Tyneside Council is committed to safeguarding and promoting the welfare of children, young people and vulnerable adults. We expect all staff and volunteers to share this commitment and an Enhanced Disclosure and Barring Service (DBS) disclosure will be sought along with other relevant pre-employment checks.
