With a low level of delayed transfers of care we are keen to lead the way in terms of innovation that will see us maintain this level of performance.

Providing both robust leadership and management of our Hospital Discharge Team within South Tyneside District Hospital, you will co-ordinate and affect timely discharges for patients with ongoing health and social care needs. You’ll have a successful background of multi-disciplinary and partnership working which will build on the excellent joint working relationships already established with health colleagues.

Candidates will need a deep understanding of excellent social work practice, and the rigour and management grip to work within the complexities of a multi-disciplinary hospital setting. You will be innovative and creative, open to ideas and committed to individual learning and development.
