Stockton-On-Tees Adult Services has an opportunity for a fixed term, 12 month, Social Care Officer post to join their Older People Mental Health Team within Adult Services.

We are looking for experienced, committed and enthusiastic individuals who can demonstrate and undertake Assessment processes, Support Plans and associated co-ordination of social care aspects. Successful candidates are expected to have the skills to ensure that the packages received are appropriate to the needs of the clients and that identified outcomes are being met.

You will be qualified to NQF Level 3-4 in a related subject area, or the equivalent level of knowledge gained from demonstrable directly relevant works experience. You should have a general understanding of the legislation and good practice that underpins adult care, in particular the Care Act (2014), Mental Capacity Act (2005); and Making Safeguarding Personal. You should be confident in using ICT and Word, Excel and Outlook (Microsoft). You should have experience of working directly with adults and professionals in a health or social care environment and knowledge and experience of a person-centred approach to assessment and support planning for adults to maximise their independence and support requirements.

This role requires flexibility to work contracted hours over a 7-day period from 7am-7pm, to meet the needs of the service.

Government legislation stipulates that all persons working within a care home must be fully vaccinated for COVID-19 (subject to medical exemption) from 11th November 2021.
