Required to cover Maternity leave
To work as a team member within the Portage and Pre-School Service.
You will provide a Portage home visiting service to families/carers of pre – school children with SEND and when appropriate provide monitoring visits during the transition period from home visit to an early years setting.
The postholder will support the child’s individual additional needs in the home implementing and modelling strategies working in partnership with the family and other agencies/services to ensure the best outcomes for the child.
