This is a great opportunity to join the Recovery and Independence Team at an exciting time of development and expansion. The Recovery & Independence Team provide:

- Reablement Service to adults supporting them to learn or re-learn skills of daily living with the aim of them reaching their potential to live independently in their own home.

- Rapid Response Service providing support (within two hours) with personal care, meals and medication to prevent inappropriate emergency admissions to Care homes or hospital and support hospital discharge.

The team covers the whole of Redcar and Cleveland Borough offering the service 7 days per week, from 7am to 10pm. This post-holder will be required to work Monday to Friday 8:30-5 and participate in a weekend working rota currently one weekend in six and an out of hours on call system.

As the service continues to expand and develop, we are looking for two additional Co-ordinators to assist with staff line management responsibilities including; supervision and appraisal and absence management, the initial assessment for re-ablement, the monitoring and reviewing of individual programmes of support, referrals onto other services, assessment of the individuals on the rapid response service in relation to their potential to be re-abled or requirements for on-going needs.
