Hartlepool Borough Council are seeking to appoint 1 Family Support Worker to support families where an Early Help assessment has identified 'need' and a Team Around the child approach is required.

The successful candidate will be based in one of 4 Early Help Locality teams in Hartlepool, supporting families where 'need' has been identified via the completion of an assessment. While working with the family and other professionals, the candidate would ensure that everyone is working toward the goals set by the family.

This is continuously monitored through reviewing the family plan. The candidate will have excellent communication skills, the ability and confidence to work alongside families and to take on a Lead Practitioner role.

The successful applicant will be required to undertake an enhanced check via the Disclosure and Barring Service and a satisfactory health screening questionnaire.

It is essential that applicants have NVQ II in relevant area or working towards NVQIII in the relevant area or equivalent.
