Mental Health Support Worker
To support young people who are facing homelessness and need extra support around mental health issues. To work with young people to identify and address mental health issues with the aim of improving their mental health and their engagement with mental health services.
Main Responsibilities and Duties:
Project Delivery
• To support a caseload of young people with a different range of mental health issues including not yet identified
problems, issues in accessing and engaging with other mental health services and support to transition from Children
and Adolescent Mental Health Service to Adult Mental Health Services
• To develop and coordinate a range of group activities and sessions to support young people to have more
self-awareness around mental health
• To liaise with other agencies and partners involved in supporting young people who are impacted by homelessness,
and who have mental health needs
• To monitor and evaluate the provisions and review the progress made by young people using a variety of methods.
